The IUOE Local 57 Health & Welfare Fund provides a variety of welfare benefits to its participants and their dependents which include Medical, Dental, Vision, and Life Coverage. How to Become Eligible for Benefits
You are eligible for Medical, Dental & Life Insurance on the first day of an Insurance Period immediately following:
-The Eligibility Period in which you have accumulated at least 500 hours of Covered Employment, or
-a 12-month period in which you have accumulated at least 1,000 hours of Covered Employment
whichever is sooner
An Insurance Period is a period of 6 consecutive calendar months commencing on the first day of any January or July
An Eligibility Period is a period of 6 consecutive calendar months commencing on the first day of any January or July.
