The Local 57 Health & Welfare Life Insurance Plans are currently managed through the Union Labor Life Insurance Company. For more information regarding these plans, please refer to the Summary Plan Description or contact the Benefit Funds Office. If you have a death benefit claim or if you have any questions, you may contact the Funds Office at (401) 331-9191, weekdays 8:00am-4:00pm.

The IUOE Local 57 Life coverage is covered by Union Labor Life Insurance Company. If you die from any cause while you are insured, the proceeds, as shown below, will be paid to your beneficiary:

Active Employees (who die on or after July 1, 2012): $40,000

Retirees (who die on or after August 1, 2010): $5,000

Active employees who die on or after July 1, 2012 have Accidental Death & Dismemberment coverage for up to $20,000.
